Improve plugin readmes
Now every plugin has a short, introductory readme tiddler that is shown
in the online plugin library.

Deprecate $:/tags/stylesheet in favour of $:/tags/Stylesheet
A warning message appears at the top of the story river if any
`$:/tags/stylesheet` tags are found.
