Fix Bootstrap 5 modal-header order and dead BS3/4 classes
...
The Book Details and Fetch Metadata modal headers had the close
button before the title in the DOM, so Bootstrap 5's
`.modal-header { justify-content: space-between }` pushed the title
to the right and the close button to the left. Reorder title before
close button to match every other modal in the app.
Fetch Metadata's modal also still used leftover Bootstrap 3/4
markup with no matching CSS in the BS5 build (`class="close"`,
`sr-only`, `input-group-btn`, the "media object" result-item
pattern, `glyphicon-ok`), and had its search form/hint text
sitting inside `.modal-header` instead of `.modal-body`. Replaced
with Bootstrap 5 equivalents (`btn-close`, `visually-hidden`, flex
utilities, `bi bi-check`) and moved the form/hint into the body.
