Jeremy Ruston df80a42d13 Restrict filter pragmas to the start of a filter ...

Pragmas appearing after a filter run now return an error. This removes the
ambiguity where the subfilter and filter operators saw the final pragma value
regardless of their position. Adds generic docs for filter pragmas and an
impact note for titles beginning with double colons
